There are three ways to apply for Coventry health insurance.  You can APPLY ONLINE, fax or mail the application to us. 

 

If you are faxing or mailing, fully complete, sign, and date the APPLICATION.  Indicate the plan you have selected by checking off the appropriate box or boxes under Part A, Benefit Selection.  Indicate your estimated monthly premium.  Fax the application to us at 678-475-3851 or mail it to us at 2405 Satellite Blvd Ste 200, Duluth, GA  30096.  If you fax the application to us you must choose Bank Draft as your option for payment.  If you wish to pay by check, mail the application along with a check made payable to Coventry Health Care of Georgia, Inc.  Any questions you have regarding enrollment, please call us directly at 678-475-5709 or email us at info@mccartinsurance.com.
