Eligibility for CoventryOne Health Insurance
Eligible persons are individuals under the age of 65 who are not eligible for Medicare. Eligible dependents include children under age 19 as well as dependent unmarried children between 19 and 25 who attend school on a full time basis. All eligible persons must reside in the service area. Please refer to the service area map.
Enrolling for CoventryOne Health Insurance
There are three ways to apply for Coventry health insurance. You can APPLY ONLINE, fax or mail the application to us.
If you are faxing or mailing, fully complete, sign, and date the APPLICATION. Indicate the plan you have selected by checking off the appropriate box or boxes under Part A, Benefit Selection. Indicate your estimated monthly premium. Fax the application to us at 678-475-3851 or mail it to us at 2405 Satellite Blvd Ste 200, Duluth, GA 30096. If you fax the application to us you must choose Bank Draft as your option for payment. If you wish to pay by check, mail the application along with a check made payable to Coventry Health Care of Georgia, Inc. When Coverage Begins with Coventry Health Care of Georgia
The earliest coverage can begin is the 1st or the 15th of the month following underwriting review and acceptance of the application. Allow a minimum of 15 days to review and process your application. Applications for coverage may be accessed an additional premium or denied based upon the health status of the applicant.
